I scanned the documents they sent me into a PDF with everything necessary for Irish citizenship (PDF, 9.6 MB). Page 5 is the form to request a copy of the birth certificate of your grandparent, showing that they were born in Ireland. Pages 7 and 8 are the form to register your own birth and therefore gain citizenship after you've gathered the supporting documentation.
